AFFIDAVIT OF LOSS

I, CLARIAN L. CINCO, of legal age, single, Filipino citizen, and


presently residing at Brgy. 95 Caibaan Tacloban City, after having been
duly sworn to in accordance with law, do hereby declare that:

1. I am the rightful owner and holder of a BDO ATM card, which was
officially issued under my name by Banco de Oro Unibank, Inc. (BDO)
2. On July 02, 2025, I found that my ATM card was lost, and
notwithstanding earnest efforts to trace or retrieve it, it has remained
missing and is beyond recovery.
3. The said ATM card has not been confiscated, surrendered, or
transferred to any person or entity, and at no time have I granted
authority to anyone to use or hold the same.
4. I am executing this Affidavit to affirm the truth of the foregoing
statements and for the purpose of reporting the loss and securing the
issuance of a replacement ATM card from Banco de Oro Unibank, Inc.
(BDO).

IN WITNESS WHEREOF, I have hereunto signed this Affidavit on
