AFFIDAVIT OF LOSS

I, RODALYN E. ARES, Filipino, married, of legal age and a


resident of Road 3 Brgy, Minuyan II San Jose Del Monte Bulacan, after
having been duly sworn in accordance with law, hereby depose and
state:

1. That, I, am an Account Holder of UnionBank of the


Philippines at Baesa Caloocan Branch;

2. That, I, am the lawful owner of the lost ATM Card with


Account Number: 109651013392;

3. That, unfortunately, last March 09, 2021, I lost my wallet with


my ATM Card when I, was at riding a jeepney along Sapang Alat North
Caloocan;

4. That, after a thorough search and despite diligent efforts to


locate, I can now conclude that these documents are irretrievably lost;

5. That, I am executing this Affidavit to attest to the truth of the


foregoing statements, and for whatever legal purpose or intent that
this may serve.
