 Serious gram-negative bacterial infections (e.g., Pseudomonas, Klebsiella, E. coli,


Proteus, Serratia)
 Septicemia
 Bacterial endocarditis
 Complicated urinary tract infections
 Intra-abdominal infections
 Respiratory tract infections (especially hospital-acquired pneumonia)
 Skin, bone, and joint infections
 Often used in combination with other antibiotics for synergistic effect

Nursing Responsibilities

1. Assessment & Monitoring


o Check baseline renal function (BUN, creatinine) before starting therapy
o Monitor peak and trough serum gentamicin levels to avoid toxicity
o Assess for hearing changes (risk of ototoxicity)
o Monitor urine output (nephrotoxicity risk)
o Monitor infection signs (temperature, WBC count)
2. Administration
o Give exact prescribed dose; avoid exceeding recommended duration
o Dilute for IV infusion and administer over the recommended time
o Ensure adequate hydration to protect kidneys
3. Patient Education
o Report ringing in the ears, hearing loss, or balance issues immediately
o Report changes in urination or swelling
o Complete the full course even if symptoms improve
4. Safety Precautions
o Avoid concurrent use with other nephrotoxic or ototoxic drugs (e.g.,
furosemide, amphotericin B) unless necessary
o Use cautiously in elderly and those with kidney disease
