I, LAVENDER ANYANGO MAINA a holder of Identitification Number 40996449 and P.O BOX 305
RONGO, within the Republic of Kenya do swear this affidavit and state as follows :-
1. THAT I am a female adult person of sound mind hence competent to swear this affidavit.
2. THAT I am the deponent in this matter.
3. THAT I registered for my NATIONAL IDENTITY CARD my name appearing as LAVENDER ANYANGO
MAINA,but in my registration by the ministry of lands for the purpose of title deed ownership, an error
occurred and my name was registered as LABENDA ANYANGO MAINA.
4. THAT I now wish to state that LAVENDER ANYANGO MAINA as it appears on my NATIONAL
IDENTIFICATION CARD and LABENDA ANYANGO MAINA as it appear on my TITLE DEED, refer to one
and the same person which is me.
5. THAT I wishes to state that in absence of any correction made on the above stated error, that I am
identified by both names.
6. THAT what is stated herein above is true to the best of my knowledge, information and belief.
