I, ISABEL R. TAGLE, of legal age, Filipino, widow, and a resident of 38 S.
Aguirre Street, Mulanay, Quezon after having been duly sworn to in accordance with
law, do hereby name, constitute, and appoint FELOMINO R. TAGLE/ Attorney-in-
Fact, of legal age, Filipino, and a resident of 38 S. Aguirre Street, Mulanay, Quezon,
to be my true and lawful Attorney-in-Fact, for me and in my name , place, and stead,
to do and perform the following acts and deeds:
1. To appear, transact, and represent me before the Government Service
Insurance System (GSIS) or any concerned agency or institution, in
connection with the filling, processing, and claiming of all applicable benefits,
claims, and entitlements arising from the death of my daughter, JULIE R.
TAGLE, who was a GSIS member at the time of her death;
2. To receive, sign, and acknowledge any and all documents, checks, or benefits
that may be issued in my favor, including but not limited to survivorship
benefit, funeral benefit, or any other applicable claims;
3. To sign, execute, and deliver documents and papers necessary to implement
the authority herein granted; and
4. To do all other acts necessary and related to the purposes above stated.
HEREBY GIVING AND GRANTING unto my said Attorney-in-Fact full power and
authority to do and perform all and every act and thing whatsoever requisite,
necessary, or proper to be done, as fully to all intents and purposes as I might or
could lawfully do if personally present, hereby ratifying and confirming all that my
said Attorney-in-Fact shall lawfully do or cause to be done under and by virtue of
these presents.
