LASIEC ELECTORAL FORMS
There are thirty (30 Nos) various forms being used by the Commission
in the course of conduct of Local Council Elections in Lagos State.
These forms are used at different stages of election but generally
these forms can be grouped into four categories:-
(i) Nomination forms - Nine forms (9 No’s);
(ii) Result sheets –Seven forms (7 No's);
(iii) Confirmation forms – Nine forms (9 No’s);
(iv) Oath of Allegiance/Affirmation form – One form (I No)
NOMINATION FORMS
(a) LASIEC 001 - Data Forms for person seeking election into
LGA/LCDA.
(b)LASIEC 002 - Submission of List of Candidate by Political Party.
(c) LASIEC 003 - Return of List of Candidates verified to contest
election.
(d)LASIEC004 - Return of List of Candidates declared not qualified to
contest
Election.
(e) LASIEC 005 - Appeal against Non-Validation of Candidates.
(f) LASIEC 006 - Electoral Officer’s Ruling as to the Validity of
Nomination.
(g)LASIEC 007 - Nomination of Candidates for Chairmanship Election.
(h)LASIEC007A - Nomination of Candidate for Vice Chairmanship
Election.
(i) LASIEC 008 - Nomination of Candidate for Councillorship Election.

From the foregoing, it could be seen that out of the thirty (30) Electoral
Forms being used by the Commission, Presiding Officers make do with ten
(10 Nos) out of the most sensitive of these forms, thereby, making the job of
the Presiding officers most sensitive and paramount to the conduct of Local
Council Elections.
In view of the above, training and re-training of these categories of poll
officials become imperative in order to make them well acquainted with their
responsibilities before, during and after Election.
