INSTRUCTIONS FOR REGISTRATION

Produce following Original certificates for verification along with their attested photocopies and Prescribed
Application Form :

Apply in Prescribed application form within 3 months from the expiry date of Provisional Certificate
certificates for verification.
Proof of date of birth (One of the following)

 School leaving Certificate


 Birth Certificate
 SSC/HSC Passing Certificate of Board
 Passport

M.B.B.S. Degree Certificate issued by university


Certificate of satisfactory completion of internship

 Issued by the Head of the Institution Annexure I & II in original.


 Issued by the University

Proof of change of name in case of married woman desirous of registration in new name -

 Registration of marriage certificate


 Any one – i) Govt. gazette ii) affidavit with photograph

Three copies of latest passport size photographs.


Certificate of Provisional Registration in original issued by MMC
Demand Draft of Rs .2000/- (Rs. Two thousand Only) in favour of Registrar Maharashtra Medical
Council payable at Mumbai.
Those who have applied after 3 months completion of internship he/she has to submit an affidavit
with photograph of Rs.100/- stamp paper mentioning the reason of delay with proof.
