NOTICE FOR RENEWAL OF REGISTRATION

1. All Registered Medical Practitioners are required to renew their

registration before the last date of validity of their registration.

2. The renewal of registration is required to be completed online before

the due date.

3. After, completing online renewal procedure and online payment

submit the following documents by Post/Courier/Personally to

Maharashtra Medical Council office in Mumbai.

Signed original Renewal Application Form.

Signed original Undertaking Certificate.

Two Passport size Photographs.

Self attested photocopies of relevant CME Certificates.

Self attested photocopies of Permanent Registration

Certificate and Additional Qualification Certificate.

4. Only after receipt of your documents at Maharashtra Medical Council

office, your renewal will be deemed to be complete. Therefore you

must submit the documents mentioned above to Maharashtra

Medical Council office.
