WAIVER
SANGGUNIANG KABATAAN NG BARANGAY CORPORACION
LINGGO NG KABATAAN 2025 – OUTING
I, the undersigned parent/guardian of __________________________________ (Name of Participant), a
resident of Barangay Corporacion, hereby give my full consent and permission for my son/daughter/ward to
participate in the Linggo ng Kabataan (LNK) 2025 Outing organized by the Sangguniang Kabataan ng Barangay
Corporacion.
The event will be held on August 31, 2025 at 2:00 PM until September 1, 2025 at 12:00 NN in Mati City,
Casa Blanca Privat e Resort. During the outing, the organizers will provide food, drinks, games, transportation,
and accommodation for all participants. For transportation, the SK has prepared “bonggo” units for everyone to
ride together, although participants may still use their own motorcycles if necessary, with the advice that group
travel is preferred for safety reasons. As for accommodation, there are limited beds available, good for only
about 20 to 30 persons, so all participants are encouraged to bring their own personal beddings, tents, or
duyan. To lessen waste and ensure personal responsibility, participants must also bring their own utensils as
these will no longer be provided.
Waiver of Liability:
I fully understand that the SK Council and Barangay Corporacion will take all necessary precautions to
ensure the safety of participants during the said activity. However, I hereby release, waive, and discharge the
Sangguniang Kabataan ng Barangay Corporacion, Barangay Officials, and organizers from any and all liability,
claims, or demands for personal injury, accidents, loss, or damage to property that may arise during travel,
participation in activities, or stay in Mati City.
Conditions:
My child/ward shall follow all rules, regulations, and instructions given by the SK officials and organizers.
I understand that failure to submit this waiver means my child/ward will not be allowed to join the
activity.
I allow the SK Council to arrange transportation for early return (in case of class schedules or
emergencies).
By signing this document, I acknowledge that I have read, understood, and voluntarily agreed to this waiver.
