The unit was checked in my presence and the complaints so stated are true. I agree to the terms and conditions listed hereunder: a. C-tech Repair Shop has sole right to refuse servicing of foreign made units if the parts needed are no longer available. Service time is subject to availability parts. b. C-Tech Repair Shop shall not be answerable to the loss/damage of this unit in the event of robbery,fire,typhoon or other natural calamities. c. C-tech Repair Shop shall automatically repair my unit of the total service charge is less than or equal to ___________________ d. The customer must report the loss of claim slip or job order to C-Tech Repair Shop and secure affidavit of loss immediately. C-tech Repair Shop shall not be held liable for any circumstances arising from the said loss.

If the person who claims the unit is not the signatory on the Job Order, he/she should secure and present an authorization letter and valid Identification Card of the signatory together with his/her Identification Card and Claim Slip or Job Order. Units not claimed 30 days after the date of repair or from last communication with customer will be charged a minimum storage fee of Five Pesos (Php.5.00) per day. Units not claimed 60 days after the date of repair or from last communication with customer shall be forfeited in favor of C-Tech Repair Shop without need of notice to cover the cost of repair storage. Units will be disposed 30 days after 60 days given period to claim the unit.
