AFFIDAVIT OF CONSENT
I, MARIA RICA VISORDE VITO, of legal age, single, Filipino, and a permanent resident of #355 de Lara St., Talisay, Camarines Norte, after having sworn in accordance with law, depose and say: 1. That I am the same Maria Rica Visorde Vito who is the subject person to be adopted in a verified petition now pending before the Regional Trial Court, Branch 40, Daet, Camarines Norte, entitled: In re: In the Matter of the Petition for Adoption of Maria Rica Visorde Vito, the petitioner of which is one Lilia A. Mancenido; 2. That I was born on March 10, 1986 at Talisay, Camarines Norte and that my natural and legitimate parents are the spouses Ricardo V. Vito and Remedios M. Visorde; 3. That when I was still young, the spouses Tomas V. Mancenido and Lilia A. Mancenido of Poblacion, Talisay, Camarines Norte were fond of attending and looking after me. Most of the time said spouses Tomas and Lilia Mancenido would take me to their house and let me stay there for several days. Sometime, when my natural parents are busy with their work they allowed the spouses Mancenido to take custody of me; 4. That later on my natural parents eventually entrusted my full custody to Spouses Tomas and Lilia Mancenido. I was taken cared of and reared by Spouses Tomas and Lilia Mancenido as if I was their own daughter. That the spouses Tomas and Lilia Mancenido are respectable and decent members of the Municipality of Talisay. In fact, the late Tomas Mancenido was the former Mayor of Talisay; 5. That after the death of Tomas Mancenido on October 13, 1997, her wife and petitioner Lilia Mancenido did not give up my custody but that she continued caring and supporting me by providing me all the necessities in life; 6. That for this reason, I am giving my unequivocal CONSENT to my intended adoption by Lilia Mancenido whom I have considered to be my true mother all these years; 7. That I have not been forced, coerced, intimidated nor paid by anyone in giving my unequivocal CONSENT to the subject adoption. IN WITNESS WHEREOF, I have hereunto affixed my signature below this ____________________ at Daet, Camarines Norte.
