NCP

 Vital signs remained stable.  Patient was able to identify factors that intensify pain.  Patient verbalized relief of pain after taking medication. The goal of lowering the pain scale to 3 was achieved. Independent:  Patient was able to verbalize factors that intensify pain. Dependent:  Medication was effective in relieving pain. Nursing interventions were effective in achieving the desired outcome.
