AFFIDAVIT OF NON-COHABITATION
I, PATRICIA PACIFICO VENTURA, of legal age, Filipino, married
and a resident of Road 5 Area C, Bagong Silangan, Quezon City, after
having been duly sworn to in accordance with law hereby deposes and
states that:
1.

I am currently married to JOEL D. VENTURA;

2.

However, the above stated person and I have been separated


for almost One(1) year and Six(6) months now;

3.

That I have been living with my daughter Ma. Christina


Ventura since 2015;

4.

That the purpose of the execution of this affidavit is to affirm


the facts alleged herein and for any legal purpose it may
serve.
