AFFIDAVIT OF LOSS I, MARIA LOURDES BASCON LLANES, of legal age, Filipino, and resident of Door 1, St. Peter St., Lopez Village, San Isidro, Parañaque City, after being duly sworn to in accordance with law, hereby depose and say that: 1. I am a holder of a meter deposit receipt issued by the Manila Electric Company (MERALCO) in my favor, for service address at Door 1, St. Peter St., Lopez Village, San Isidro, Parañaque City under Service ID number 461957102-9. 2. On January 7, 2010, I discovered that said Meralco meter deposit receipt inadvertently lost. 3. Inspite of diligent efforts to look for said meter deposit receipt, all such efforts proved futile and in vain up to this time, thus, I am declaring the loss of the said meter deposit receipt. 4. I am executing this affidavit to attest to the truth of the foregoing facts and for all its legal intents, effects, consequences and purposes, including for the issuance to me of another SIRB in replacement of the one that was lost. AFFIDAVIT OF LOSS
I, GLORIA L. NERY, of legal age, Filipino, widow, and resident of 522 Mag.
Mapa St., Bacood, Sta. Mesa, Manila, after being duly sworn to in accordance with
law, hereby depose and say:
1. That an Official Receipt No. 698485922 dated March 15, 2015 in the
amount of P3,179.06 was issued for a motor vehicle bearing Plate No. XKB162
under the name of my deceased husband, Juan O. Nery, by the Land Transportation
Office (LTO).
2. That the original of Official Receipt No. 698485922 covering said
motor vehicle was inadvertently lost.
3. In spite of diligent efforts to look for said OR, all such efforts proved futile
and in vain up to this time, thus, I am declaring the loss of the said OR.
4. Said OR was never given, or allowed to be used, to anyone for any
transaction or disposition, nor was said OR the subject of any confiscation or legal
process, there being no such legal process.
5. I am executing this affidavit to attest to the truth of the foregoing facts
and for all its legal intents, effects, consequences and purposes.
