AFFIDAVIT OF DELETION

I, ROSARIO ARALAR ESPOSO, of legal age, Filipino


Citizen, Single and presently residing and with postal
address at 3020 2nd Street, Guadalcanal, V.
Mapa, Sta. Mesa, 1016, after having been duly sworn to in
accordance with law, do hereby depose and state:

1. That I am a SOCIAL SECURITY SYSTEM (SSS)


Member under SSS No. 03-450537504;

2. That my SSS Records shows that I was employed


with Pacific Banking Corporation;

3. That I was never employed nor connected in any


capacity with the afore-said Pacific Banking Corporation
and that my only employment in the past was with INTEL
PHILIPPINES MFG, INC., UNIVERSITY OF STO. TOMAS, main
Library and the Printing Office of the same University;

4. That in light of the foregoing, I most respectfully


request the SSS Office to delete or cause the deletion of
my employment with the afore-said Pacific Banking
Corporation from my SSS Records.
