1. I am a pension A/c holder in your branch drawing pension in


A/c No-11345356045 w.e.f. Jan 2000.
2. Presently I wish to change my A/c No from -11345356045
to 36404022714 due to forgery of Rs. 90,000/- from my A/c
on date 27th Dec 2016.
3. I had open the new A/c in your branch with A/c No-
36404022714 and had already closed my old pension
beneficiary A/c after forgery.
4. You are requested to change my pension A/c to mu new A/c
No-36404022714, so that my pension and left over fund
may be credited as early as possible.
