Tourniquet Test

Common Pediatric Procedures

A. TOURNIQUET TEST or Rumpel-Leede or Capillary Fragility Test

Indications: determine hemorrhagic tendency; defined by WHO as one of the requisites for
diagnosing Dengue Hemorrhagic fever

Disadvantages: has low specificity; maybe (+) in other viral infections (eg. acute gastroenteritis);
a (-) test may occur in early stage of dengue fever.

Procedure:

1. BP cuff is inflated at a point between SBP and DBP (mean BP equals SBP plus DBP, then
divide by 2) above the cubital fossa for 5 minutes.

2. After release of the cuff, the number of petechiae within 1 square inch below the antecubital
fossa is counted

3. >20 petechiae is abnormal; indicative of thrombocytopenia or platelet dysfunction or capillary


fragility
