AFFIDAVIT

I, X, 24 years old, single, Filipino citizen, and a resident of


CCC, after having been sworn in accordance with law, hereby
depose and say:

That I am applying for issuance of a Philippine Passport with


the Department of Foreign Affairs in Cagayan de Oro City;

That in support of such application, I was required to


present certain documents as proof of my personal identity,
among which are Birth Certificate and Baptismal Certificate;

That in my Birth Certificate, my family name was spelled as


AA. However, it was erroneously entered in my Baptismal
Certificate as BB;

That despite this discrepancy, I hereby affirm and confirm


that the true and correct spelling of my family name is that which
appears on my birth certificate which is AA, and I intend to use
this spelling in my Philippine Passport for which I request issuance;

That I execute this affidavit freely and voluntarily to attest to


the foregoing facts and for the purpose of complying with the
requirements of the DFA.

FURTHER, Affiant sayeth none.

IN WITNESS WHEREOF, I hereunto set my hand this 18 th


day of March 2015 at Cagayan de Oro City, Philippines.
