Taxpayer Identification Request

Dear _______ / To Whom It May Concern:

We are requesting your assistance in providing us with the information necessary to process payments
to you. The Internal Revenue Service requires us to receive and retain a Form W-9 from you to certify
your name, address, U.S. residency status, taxpayer identification number, and backup withholding
status. We only use your tax information for required governmental reporting purposes. For your
convenience we have attached a Form W-9 and you can also find a Form W-9 online at
http://www.irs.gov/pub/irs-pdf/fw9.pdf.

We are unable to process payments to you until we receive a completed and signed Form W-9. Without
the Form W-9 certification payments may be subject to withholding. Failure to provide accurate
reporting information on a Form W-9 may result in the IRS imposing penalties on you as the taxpayer.

If you are not a U.S. person (citizen or permanent resident) you should not complete a Form W-9.
Instead, please complete a Form W-8 BEN, available online at http://www.irs.gov/pub/irs-
pdf/fw8ben.pdf. Please note that different tax withholding and reporting rules may apply.

To allow us to process any pending and future payments, please return a completed Form W-9 or W-8
BEN as soon as possible via email to EMAIL@byu.edu, via fax to 801-422-####, or via postal mail to:
