Subject: Explanation Letter

Dear Ma’am Saniel,

Good day!

Referring to the above matter, I’m Arfe M. Silvano, BS Nursing fourth year student. I
was assigned to be part of the badminton sport medics’ team during this year’s
intramural days. Due to the following reasons I was not able to participate on the
previous medic days including last Wednesday August 23, and the previous ones,
August 9 and 16, 2017.

During August 9, 2017- Opening of the intramurals day, I asked one of the players of
my assigned sport and was told that some of the games had not yet started including
theirs, therefore my medical assistance was not yet in need.

By the next Wednesday, August 16, 2017, games has already begun, and I was on
school but decided not to participate in medics. I prioritized my time on finishing our
research proposal and preparing for the presentation by next day, consuming my
whole day doing that process.

And by this week’s Wednesday, August 23, 2017, I had yet made another absence on
medics due to the fact that I was not been able to wake up early in morning, more
likely it was by near lunch time that I was able to be out in bed, a result made by my
tiresome body due to the sixteen hour hospital duty the day before, from 7 am in the
morning till midnight of August 22, 2017. Being not participated in morning I then
decided to get my NC-II Healthcare Certificate on TESDA office on the afternoon.

With the above explanation being said, I am very much sorry for the absences that I
have made during my medic days, and in return I humbly accept any penalties that I
will be facing against my actions.
