(ACR I-CARD)

I, MR. ADAM LEE GEORGE KNOX, a British national and holder of a United Kingdom of Great
Britain and Northern Ireland passport, respectfully request for the issuance by your good office of a
waiver of ACR I-Card.

I was hired as Brand Ambassador (for The Dalmore) by EMPERADOR DISTILLERS, INC. (“EDI”), a
Filipino corporation engaged in the business of manufacturing, importing and exporting of liquor and
wine products.

Concurrently, I have already acquired my 9(g) visa for my working permit. Unfortunately, one of
the requirements by your office before leaving the country is an ACR I-Card. However, it still pending on
your office and I need to go outside the country for business purposes. Considering that my ACR I-Card
was still pending may I request a waiver of the said ACR I-Card.

Attached herewith are the following pertinent documents in support of the aforesaid request
for a Waiver of ACR I-Card:

1. Photocopy of the ACR I-Card transaction form claim stub;


2. Photocopy of the passport – bio page with latest Visa implementation page; and
3. Photocopy of airline ticket.

We hope this request merits your usual prompt and favorable action.
