     1.    Petitioner, whose complete name is __________,

made an integral part hereof as ANNEX “A.”
     2.   Petitioner is a citizen of ______, a country whose
laws grant Filipinos the right to become naturalized citizens
thereof. Petitioner is registered as a resident alien and
possesses an alien certificate of registration from the

     5.   Petitioner speaks and writes English and Filipino.
      6.   Petitioner is married since ______ to ______, a
Filipino Citizen, __ years old, born on ____ at ____. Marriage
Contract. Birth Certificate of wife. Consequently,
Petitioner is entitled to the benefit of Section 3 of the
Revised Naturalization Law (Commonwealth Act No. 473)
reducing to five (5) years the requirement under Section 2
of ten (1) years of continuous residence.
      7.   Petitioner and his wife begot two children, namely
____ and ____, who were born on _____ and ____, at ______
respectively, and both raised in the Philippines since birth.
Petitioner’s children studied and graduated grade school
and high school from ______________, a private school
recognized by the Government, which is not limited to any
race or nationality, and where Philippine history,
government and civics are taught or prescribed as part of
the school curriculum. ____ and ____, now adults, are
currently residing at ____ and ___, respectively. Birth

     8.   Petitioner is exempt from the requirements of
Section    6    of    the   Revised      Naturalization   Law
(Commonwealth Act No. 473) regarding the filing with the
Office of the Solicitor-General a declaration under oath of
his bona fide intention to become citizen of the Philippines,
as he has been residing in the Philippines for more than 30
years prior to the filing of this Petition, and given primary
and secondary education to all his children in a private
school recognized by the Government and not limited to any
race or nationality.
     9.   Petitioner possesses all the qualifications under
Section 2, and none of the disqualifications under Section 4
of the Revised Naturalization Law.
           9.1. Petitioner  believes     in   the    principles
     underlying the Philippine Constitution; is of good
     moral character and has conducted himself in a proper
     and irreproachable manner during the entire period of
     his residence in the Philippines in his relations with the
     constituted government as well as with the community
     in which he lives; has mingled socially with the
     Filipinos, and has evinced a genuine desire to learn

     and embrace the customs, traditions, and ideas of the
     Filipinos.
           9.2. Petitioner is not opposed to organized
     government or affiliated with any association or group
     of persons who uphold and teach doctrines opposing
     all organized government; he does not defend or teach
     the necessity or propriety of violence, personal assault,
     or assassination for the success and predominance of
     their ideas; Petitioner is not a polygamist or a believer
     in polygamy or in the practice of polygamy, Petitioner
     has never been convicted of any crime involving moral
     turpitude, Petitioner is not suffering from mental
     alienation or incurable contagious disease; and
     Petitioner is not a citizen or subject of a nation at war
     in the Philippines, or a country whose laws do not
     grant Filipinos the right to become naturalized citizens
     or subjects thereof. Barangay Clearance, Police

      10. It is Petitioner’s intention in good faith to become
a citizen of the Philippines and to renounce absolutely and
forever any allegiance and fidelity to _______. Petition shall
likewise reside continuously in the Philippines from the date
of the filing of this Petition up to the time of his admission to
Philippine citizenship.
      11. This Petition is supported by the Judicial Affidavits
of two credible persons, namely, ______ and ____, both
Filipino Citizens, and residents of _________ and ______,
respectively, who personally know the Petitioner to be a
resident of the Philippines for thirty plus (30+) years, to be
a person of good repute and morally irreproachable, and
who, in their opinion, that Petitioner has all the
qualifications necessary to become a citizen of the
Philippines and is not in any way disqualified under the
provisions of the Revised Naturalization Law.
    12. Petitioner desires to become a citizen of the
Republic of the Philippines.
                           PRAYER
     WHEREFORE, Petitioner respectfully prays, after due
notice, publication, and hearing, that he be admitted as a
citizen of the Republic of the Philippines.
