NCP (Vaginal Bleeding)

1. The patient was experiencing a fluid volume deficit due to profuse vaginal bleeding after giving birth. 2. The nursing care plan aims to prevent dysfunctional bleeding and improve the patient's fluid volume within 1-2 hours through monitoring vital signs, intake and output, and evaluating bladder function to prevent urinary retention. 3. The goal of increasing fluid volume and stopping excessive bleeding was successfully met after 1-2 hours of interventions like monitoring vitals and intake/output.
