NCP Acitivity Intolerance

The patient reported feeling easily fatigued even with minimal activity due to hepatocellular carcinoma. The nursing diagnosis was activity intolerance related to an imbalance between oxygen supply and demand. Short term goals were for the patient to demonstrate increased activity tolerance as evidenced by verbalizing less fatigue, using energy conservation techniques, and maintaining stable vital signs during activities. Interventions included encouraging planned activity progression, adjusting activity levels, teaching energy conservation methods, and instructing on alternating rest with activity. The expected outcome was the patient demonstrating increased tolerance for activity after one day.
