Using the guidelines set by the Georgia Department of Education as outlined by 2016 Judy
Serritella Exemplary Library Media Self-Evaluation Rubric, I completed an evaluation of Cross
Creek High School’s Media Center. Evaluation using this rubric focused on six categories,
twenty-two indicators, and three levels of proficiency: basic, proficient, and exemplary.
I found the following results show in the table below after evaluating our library media center
based on the above principles.

Our Media center has seven standards that meet the exemplary level of performance, seven areas
in proficient and seven areas of basic proficiency. In the table below you will find an action plan
to move our seven basic proficiency areas to proficient or exemplary. There is also an action plan
for one proficient area to be elevated to exemplary. See below.
