AFFIDAVIT OF OUT TOWN REGISTRATION

I, ________________, Filipino, of legal age, married, and a


resident of ___________________, after having been duly sworn to in
accordance with law, do hereby depose and say:

2. That the fact of my birth has not been registered in the Local Civil
Registry of __________ or any other Local Civil Registry up to the
present;

3. That due to the illiteracy of my parents, it is only now that I seek


the registration of the facts of my birth;

4. That due to the distance between ____________and


______________, it would be impractical and burdensome if I
purposely go to ________ when I can cause the registration of my
Birth as migrant filer under existing laws;

5. That I execute this affidavit to attest to the truthfulness of the


foregoing statement and for whatever worth this may serve best.
