WE, ___________ and ______________ of legal ages, Filipinos, and


residents of Maraymaray, Don Carlos, Bukidnon, after having been duly sworn to in
accordance with law, hereby depose and say:

1. That we personally know BEN ULO FRANCO, who is our neighbor at


_____________ for twenty (20) years, more or less;

2. That he is close to us as he is also a friend of our family;

3. That his SURNAME is correctly stated as “_________” as appearing in his


Certificate of Live Birth issued by the National Statistics Office AND NOT
“FRONCO” as indicated in his Driver’s License issued by the Land Transportation
Office;

4. That his middle name is also correctly spelled as “______” as reflected in


his Certificate of Live Birth issued by the National Statistics Office AND NOT
“__________” as indicated in his Driver’s License issued by the Land
Transportation Office;

5. That the person in the documents bearing the discrepancies refers to ONE
and THE SAME PERSON;

6. That the discrepancy in his SURNAME and MIDDLE NAME could have been
due to an honest mistake or clerical error committed at the time it was applied for;

7. As such, we execute this Affidavit to attest to the truth of the foregoing facts
and for whatever legal purpose that this Affidavit may serve.
