AFFIDAVIT OF UNDERTAKING
I, OLIVA POMIDA OLINO, Filipino, widow, of legal age, and a
resident of Purok I, Barangay Capoocan, Calbayog City, Philippines, after having been
duly sworn to in accordance with law, hereby depose and state, that--

1. I am the legitimate/biological mother and the legal guardian of MARIA


ANTHONETTE POMIDA OLINO, 17 years of age and is a bona fide Grade
10 student of Calbayog City National High School of Calbayog City;

2. She was born on June 14, 2000 in Pasay City but was her fact of birth was not
yet registered with the Local Civil Registrar, hence, no Live Birth Certificate can
be produced;

3. As such, she remained unregistered and her Late/Delayed Registration with the
Local Civil Registrar is yet to be processed;

4. She was then allowed entry in school even without the Birth Certificate upon
showing of her Certificate of Baptism and with a condition that a late
registration of his Birth certificate be recorded;

5. I am executing this Affidavit to attest to the truth of the foregoing facts and for
whatever legal purpose this may serve;

IN WITNESS WHEREOF, I have hereunto set my hand this 23rd day of March
2018, at Calbayog City, Samar, Philippines.
