I, ANN FELIX DEADA, Filipino, of legal age, and a resident of No. 13 Apartment D
Dahlia Street, Pacita Complex I, San Pedro, Laguna, after having been duly sworn in accordance
with law, hereby depose and say:

1. That I was married to Fernando Sison Deada resident of Narra Street, Labrador
Subdivision, Binan, Laguna;

2. On October 30, 2012, because of the irreconcilable differences and after _____ years of
marriage we decided to separate ways, at present I’m living with my daughter
__________________ in San Pedro, Laguna;

3. Because of the trauma and suffering during our marriages there is no chance of
reconciling our differences and returning to the state of marriage which existed prior to
our estrangement from each other.

4. That I am executing this affidavit to attest to the foregoing facts and to whatever legal
purposes it may served;
