Dear Dr. Mejia/Sir:

This is in regard to our client, JEREMY BATINGAN, who sought your


center’s medical assistance last July 29, 2018.

According to our client, he was brought to your center for the medical
treatment of his injuries. On said date, however, Mr. Batingan was not issued any
Medical Certificate. The undersigned believed that the Medical Certificate of Mr.
Batingan is vital to his defense in a case against him. Thus, this request.

In view of the foregoing, the undersigned requests for a copy of the Medical
Certificate of MR. JEREMY BATINGAN from your institution.
