1.That i am applying for ....................................from lifestyle immigration,terrace rd talwara 144216


punjab.

2.That i am paying INR zero to the company in front for my file,and i am only paying my embassy
fees to the embassy,and if i will withdrawal my file after this affidavit signed ,i will pay INR 10000/-
(TEN THOUSANDS) to the company as penalty.

3.That,After visa i will pay RS......................................to the company.

4.That all the academic certificates/documents submitted along with my above application for
tourist visa are giving by me is genuine and true.

5.Tha additional information certificates/documents as required by embassy,will also be provided by


me and its trur and genuine in every respect.

6.That i shall be held solely and exclusively responsible to face any action under the relevant law,in
case it is established at any stage that any of the certificates /documents above is false.

7.That i agree with the company policy that i will provide genuine&original documents only without
any alteration.

8.That after visa grant i will give payment to the company according to my aggrement with in 3 days
and i am fully aware that if i will unable to give payment to the company ,company have full rights to
take legel action in the court/police station against me.

9.That i myself providing my all financial and bank documents to the company is correct.

10.That i fully understand this company only provide me service of filling the form as required by
relative embassy on our behalf.

11.That i further understand i have read over all the averments mentioned in the affidavit with my
own language and presumed with my best knowledge and belief.

12.That after read and presumed the terms and conditions of the affidavit i have put my signatures
with my own consent and without under any pressure along with my left thumb impression in the
presence of marginal witness.

13.That i further undertake i will not file any civil complaint against Lifestyle immigration in any
manner in police station or any court of law in india.
