A Little Bit of Statistics from
US National Safety Council
• During a ten minute safety speech:
– 2 people will be killed
– 350 people will suffer a disabling injury
– Costs amount to $8,400,000
• In the USA: Each hour
– 11 unintentional injury deaths
– 2,120 disabling injuries!
– Work related- 1 death per hour
– Work related- 400 injuries per hour
Unsafe Act and

Condition
Unsafe Act
• An UNSAFE ACT is a BEHAVIOR
that increases the likelihood of
injury by violating safety rules
Ex. not wearing eye protection or PPE
Examples of Unsafe Act
• Failure to wear PPEs
• Deviation from Specs/Procedures
• Haste, hurrying, running
• Inattention, indifference
• Unsafe posture or position
• Operating equipment at unsafe speed
• Using unsafe equipment; hands instead
of equipment
• Distracting, teasing or abusing other
workers
• Working on moving or dangerous
equipment
• Unsafe loading, placing or mixing
Unsafe Condition
• An UNSAFE CONDITION is an irregular
or unusual condition caused by faulty
design, incorrect construction, or an
unsafe BEHAVIOR that may lead to an
accident or injury (such as poor wire
management).

AND INFORM RIG PERSONNEL
