Off Campus Locations
In the event of emergency, off campus sites should call 9-1-1 first to request Police/Fire/EMS services, and then call COM Police at 599 or (409) 933-8599.
This form is used by employees, students, and third-parties to report incidents of discrimination, harassment, and retaliation, including: sexual harassment, sexual assault, sexual misconduct, interpersonal violence, dating and domestic violence, stalking, sex-based discrimination, discrimination on the basis of gender identity, gender expression, and sexual orientation, pregnancy discrimination, sex trafficking, human trafficking, sexual exploitation, discrimination based on race, color, age (40 years or older), religion, national origin, disability, Veteran status or retaliation for reporting a concern, filing a complaint, or cooperating with a Title IX or EEO investigation of a complaint.

All COM employees are Mandatory Reporters. Senate Bill 212 (SB 212), requires all employees (both faculty and staff) at a public or private post-secondary institution to promptly report any knowledge of any incidents of sexual assault, sexual harassment, dating violence, or stalking committed by or against a person who was a student enrolled at or an employee of the institution at the time of the incident. Any employee who becomes aware of such behaviors should utilize this form to notify the Title IX Coordinator. For more information click here. Senate Bill 212
COM policy prohibits retaliation against a student, employee or any other person affiliated with the College for reporting a concern, filing a complaint, or cooperating in an investigation of a complaint.
A report can be made anonymously, without sharing your name. Depending on the information provided, the College’s ability to respond to and investigate the report may be limited.
