Continuity

The Polaroid photos given to Mister D by the male prison guard, are all photos of women inmates posing topless in front of a wall, however the Polaroids he takes of these women are through CCTV monitors when they are brought in for the first time.
When the women are being marched into the prison for the first time and told to strip down, the order of women changes between what we are seeing and what the male prison guard is watching through the monitors.

Factual errors

At the end of the movie when Mr D is trying to escape in his vehicle he knocks over Duane trying to escape on foot. However Duane bounces off the front of the vehicle and over to the side. When Mr D reverses and again runs over Duane, half of Duane's body is lying under the car, perpendicular to the vehicle. Even if he had rolled, there is no way Duane could be in this position, especially when Mr D had reversed in a straight line.

Plot holes

There are several indicators throughout the movie that the female prison guards are in on the scandal and pass on information, however the scene where McKinney gives her lawyer a Mr D's matchbook, prompting her lawyer to visit, it's alleged that this is witnessed by the female guard standing outside, however given where she was standing, there is no way she would have been able to witness this exchange.
