Richmond Trinity has closed its doors, and now many of the staff have been shifted to James River, a rundown hospital in a low-income area. Things are made even worse when Christina and Bobbie have a run-in with the ER nursing supervisor. Tom gives up his administrative duties and dives into direct care.

Christina disagrees with the hospital's plan not to treat a heroin-addicted patient and takes her grievances to the ethics committee. An unexpected illness forces a mother to reveal a potentially life-changing secret to her adoptive son. Camille begins to build a relationship with hospital employee Marcus.

As Camille helps Miles counsel a cancer patient, Nick (Marc Anthony) undergoes a harsh interrogation about his role in the murder of Christina's assailant.

On the heels of her and Tom's (Michael Vartan) wedding, an attack in the hospital parking lot endangers both Christina (Jada Pinkett Smith) and her unborn child. And as Morrissey (James Morrison) looks to appoint a new head nurse, Steve (Adam Rayner) makes an important decision about his future.

An old friend resurfaces and Christina is determined to help her despite the insurance company's refusal to cover her treatment. Tom's growing relationship with Erin rubs Christina the wrong way, just as tension and silence build at home between Christina and Camille

Morrissey battles both Tom and Christina to reduce the hospital's budget forcing both to fire doctors and nurses. Bobbie is put in charge, as Christina battles Morrissey, and has to deal with the death of a young baby in the E.R. which upsets all the staff. Meanwhile, Camille begs her mother to let her go to Virginia Beach for the weekend.

Christina clashes with hotshot and arrogant new surgeon Dr. Phillips over his handling of her long-time young patient Amy Johnson who has been diagnosed with AVM. Although the surgery goes well Morrissey is far from amused with her actions. Meanwhile, Amanda checks in for biopsy surgery with Tom who hires Ray to be her nurse and for the procedure to be kept from Christina. Elsewhere, Larry from Accounting overpays Candy so she suggests drinks - Larry is delighted until Ray confronts him. Who will win?
