You shouldn't doubt it, he just photoshopped out part of the title. It actually says "How to live with the regret of knowing you have a large penis inserted into your ass every night." Although I'll give him credit, it's almost impossible to see the photoshop work in that picture.

Folks always wish for a big penis, but there was a show that interviewed a gay guy that had a 13" johnson. He said it was really a bit of a curse. Some relationships with partners ended, b/c he was too big for them. He would have to get with other partners and they'd have to go through prep to get ready for him b/c he was so huge. Since it required a lot of blood to keep erect, it was sometimes difficult to maintain an erection. B/c he had such a big johnson, a lot of folks just focused on that and neglected him as a person; they just wanted him to jerk off for them or treated him like a circus side show performer.
You should be careful of what you wish for. It might come true.

^^^^ I have to agree with the poster above... A member of my immediate family was cursed with a very large one, not only lengthy but abnormally thick as well.
To top it off, he is generally attracted to petite females, so we can guess with almost certain accuracy that he goes home frustrated most of the time. Usually he will have to pay a pro to accommodate him but he isn't attracted to those types of females so again he winds up frustrated.
He's told me that many will attempt trying to accept him and he says that if they are willing to go for a marathon session that he can enter them partially but more often he winds up with the old faithful hand job.
He's considered argumentation, but he has been advised that most likely he will be reduced but loose most of the feelings and that doesn't interest him. So he is stuck with his curse.
