PETITION
COMES NOW, Petitioner unto this Honorable Court, most
respectfully avers that:
1. She was previously granted an Appointment for Notary Public
dated 24th August 2018 signed by 2nd Vice Executive Judge
Richard D. Mordeno. Copy of the Appointment is hereto
attached and marked as Annex “A” and made an integral part of
this Petition;
2. She is a citizen of the Philippines, married, born on July 1990 at
Cagayan de Oro City, with residence at Cagayan de Oro City,
Misamis Oriental, and with the regular place of work at Cagayan
de Oro City;
3. She completed her Bachelor of Laws degree Cagayan de Oro
City in the year 2016;
4. She is a member of the Philippine Bar, having been admitted on
2017 and was assigned with Roll No.. Copy of the Certificate of
Membership to the Philippine Bar dated 04 October 2018 is
hereto attached and marked as Annex “B” and made an integral
part of this Petition;

5. She is a member of good standing of the Integrated Bar of the
Philippines, as evidenced by a Certification of a Member of
Good Standing issued by two (2) executive officers of the local
chapter of the Integrated Bar of the Philippines, and the
Certificate of No Pending Case from the Supreme Court. Copies
of which are hereto attached as Annex “C”, Annex “D” and
Annex “E” respectively, and made an integral part of this
Petition;
6. She has duly paid her IBP dues being a member of the Integrated
Bar of the Philippines – Misamis Oriental Chapter as evidenced
by IBP Official Receipt No.. Copy the receipt is hereto attached
as Annex “F” and made an integral part of this Petition;
7. She has paid her Professional Tax for the current year and was
issued Professional Tax Receipt (PTR) No. dated 09 January
2018. Copy of the receipt is hereto attached as Annex “G” and
made an integral part of this Petition;
8. She has attended the Mandatory Continuing Legal Education
(MCLE) and was issued a Certificate of Compliance on 03
November 2017 for MCLE Compliance No. VI –valid until 14
April 2022. Copy of the certification is hereto attached as Annex
“H” and made an integral part of this Petition;
9. Her passport-size unretouched color photographs with her
specimen signature at the bottom part of each photograph, and
true impressions of her dry seal, duly attached respectively as
Annex “I”, Annex “J”, and Annex “K” are also made an
integral part of this Petition;
10. She has never been convicted of any crime involving moral
turpitude before any court or tribunal in the Philippines. Copy of
the NBI Clearance is hereto attached as Annex “M” and made
an integral part of this Petition;
11. She has paid the required docket/filing/application fees for the
instant Petition for Notarial Commission;

12. She has not been appointed Notary Public in any other
jurisdiction for the term applied for;
13. She has submitted her complete notarial records for the term of
the previous notarial commission in the Office of the Clerk of
Court, Regional Trial Court of Misamis Oriental; and
14. She undertakes to immediately and promptly inform this
Honorable Court of any change in her address or contact number
and to submit or report to the Office of the Clerk of Court copies
of all deeds notarized by her.

WHEREFORE, Petitioner most respectfully prays that she be
reappointed and recommissioned as Notary Public for and in the City of
Cagayan de Oro and in the municipalities/cities in the Province of Misamis
Oriental.

2. I have caused the preparation of the foregoing Petition;
3. The allegations therein are true and correct based on my personal
knowledge and authentic records;
4. To the best of my knowledge, there are no other pending petitions of
the same nature before any court, tribunal or quasi-judicial agency;
and
5. Should I thereafter learn of any similar case pending before any other
court, tribunal or quasi-judicial agency, I shall promptly notify this
court within five (5) days from discovery.
