AFFIDAVIT OF WITNESS

I, Daario Naharis, 30 years of age, Married, resident of Essos, after having sworn in
accordance with law, do hereby depose and state THAT:

01. I am an employee for 5 years of ABC BANK, as a Senior Credit Approval


Manager
02. As part of my job description, I review and approve credit card applications.
03. I have encounter and approved the application of Spouses Jane and John
Doe from January of 2016
04. That before approval of the spouses’ application, I have come to know that
the spouses have an eight year savings account with the bank.
05. That on May 2017, I received a call from John Doe, asking for an approval of
overriding his credit limit due to an urgent family matter.
06. That upon asking what his urgent Family matter was he said that his wife
Jane is in a hospital and is already need to be discharged.
07. That upon verifying the facts from Citadel Medical Center, I immediately
approved the overriding his credit limit of John Doe.

I executed this affidavit to attest the truthfulness of the foregoing facts and to support
the filing of Collection Cases against John and Jane Doe for failing to pay the amount
due on their credit cards.
