AFFIDAVIT OF DISCREPANCY

I, <NAME>, of legal age, Filipino, single and a resident of <address>, after


having been duly sworn to in accordance with law, hereby depose and say that:

1. I am the sister of <NAME>;

2. In the Register of Births of the Office of the Civil Registrar, Matanao Davao
del Sur, our mother’s first name was entered as “ROSALINDA” which
should be “ROSALINA” as her true and correct first name, as reflected in
my Certificate of Live Birth;

3. Thus, the maiden names “ROSALINDA O. ANINAO” and “ROSALINA O.


ANINAO” refer to one and the same person as our mother;

4. I am executing this affidavit to attest to the truthfulness and veracity of the


foregoing statement, accordance to the best of my knowledge,
understanding and belief, and to inform the proper authorities concerned of
the matter and for such other legal purpose/s this may serve.
