AFFIDAVIT OF LOW INCOME AND NON-FILING OF


INCOME TAX RETURN

I, BIENVENIDO T. DONATO JR., of legal age, married, and with


residence at 904 Ochids St. Punta Sta. Ana, Manila, after being duly sworn in
accordance with law, do hereby depose and say:

1. That I have an income in the amount of Three Thousand Pesos (Php


3000.00) per month as extra driver.

2. That my said work as stated above, as well as lack of livelihood and


other source of income, is not sufficient to sustain my family;

3. That as a consequence, I do not report any income and thus I never


executed or have otherwise submitted any Income Tax Return (ITR);

4. That I am executing this Affidavit in order to establish and attest to the


truthfulness of the foregoing statements, for the purpose of submitting
the same as a requirement for entrance examination of our son, ALVIN
R. DONATO, at Don Bosco Technical School, and for all other legal
purposes relative thereto.

IN WITNESS WHEREOF, I have hereunto set my hand this 25 th day of


April, 2012, in Manila, Philippines.
