1.0 SCOPE
This method statement is made to enumerate the fabrication and fixing
procedure of formwork in concrete construction in AR-RAZI Plant –V
Methanol Project, Jubail.

Method Statement A written document that explains the design and
construction at a various stages of the
work and to assure that forms are fabricated in
accordance with ACI 347 and as per latest issued
drawings and other related references.

The latest revision of the following Project Specification shall be used as
references and is part of this Method Statement in the execution of work.

4.0 GENERAL
Formwork for concrete shall be constructed to meet the requirements of the
structure and the required finish of the concrete. It shall be constructed in a way
that the concrete can be properly placed and strong enough to withstand the load
and shall conform to the required shape, dimension, level, line and position while
at use.
Formwork shall be strong, stable and rigid to withstand the pressure of fresh
concrete during the pouring and to keep or maintain its position and shape.
The number of uses of any particular formwork shall be such the quality of the
specified finish is maintained. An approved form release agent shall be used to
facilitate the removal of forms and like wise to ensure a good concrete surface
finish.
Formed surface shall be smooth finish unless otherwise noted.
Wood, generally laminated plywood and good quality timber, shall be used for
formwork in this project. The material shall be such that would yield the required
finish of formed concrete. Form ties to be used in securing and maintaining the
forms to position shall be tie rods, clamps, form jacks or wood, or any combination
of which that is suitable depending in site condition.
Forms shall be SEALED so as to prevent leakage particularly from the bottoms
and sides.
a) Forms shall be cleaned of all debris and unwanted materials prior to pouring.
b) Where concrete is to be poured onto already poured concrete, such as a
pedestal on top of base, concrete shall be “SCABBLED” to expose the
aggregate.
c) Where concrete is to be poured onto concrete, the already poured concrete
shall be soaked prior to pouring.
d) At waterstop areas a suitable “BONDING AGENT” shall be used to ensure
adequate concrete bonding.

e) At all construction joints a “KEY” shall be detailed.
f) There shall be a MEMBRANE placed under all concrete placed at grade.
g) All membranes shall be DUCT BRAND TAPED (Grey cloth tape 50mm wide)
at joints with an overlap of 150 min.
h) Where there is tear or hole damage in membranes these shall be repaired
using the DUCT TAPE.
i) There shall be a MINIMUM of TWO vibrators at every pour and these shall be
tested prior to the pour.
j) Where the concrete is to be poured or pumped into a tall form, ALL PUMPNG
HOSE ends shall have a FLEXIBLE or SOFT HOSE fitted so that the concrete
does not fall more than 1.5 max.
k) An APPROVED bituminous sealer shall be placed at the concrete to
membrane joint and this shall be used with the appropriate PRIMERS in all
cases.
l) Where CONCRETE COATINGS are to be applied concrete is to be
MECHANICALLY prepared to remove laitance as required by the coating
manufacturers.
m) Concrete is to be inspected prior to coating and repairs are to be carried out
using approved repair materials only.
n) Where there are VOIDS or CRACKS, AR-RAZI inspection is to be notified in all
cases.
o) CONCRETE CURING SECTION:
All concrete is to be cured for a MINIMUM of 14 days from the date of the pour.
Curing is to include HESSIAN BAGGING and POLYETHELENE sheeting.
Curing is to be continuous and is not to be INTERRUPTED so as to allow the
concrete to dry out.
Concrete CURE Cards are to be at every pour with the date of pour and the
date of cure end and the date of form stripping on the card.

p) Cover is to be as required by the specifications and as shown in the drawings.
q) Steel to form cover is to be as required and this is to be measured from the
closest steel (either stirrup of smallest steel closest to the form) BUT in no
case is the MINIMUM cover to be reduced, Where cover cannot be achieved
by the form geometry the form is to be moved to achieve the cover or the steel
replaced or removed.

Site Manager
The Site Manager shall be responsible for the over-all direction and monitoring of
coordination and control of all the construction activities attached and related to the
formwork.
Construction Manager
He shall be responsible for the over all implementation and monitoring of all the
activities involve in the form works.
QA/QC Inspector
The QA/QC Inspector shall be responsible for the necessary inspections and tests
to be carried out for all the activities involve in the formwork.
Field Supervisor
He shall be directly involved in the planning and execution of all activities related to
form work. He shall control the proper use and appropriate cutting of timbers to
avoid or minimize wastage of materials. He shall be responsible for making request
for purchase of materials.
Carpenter Foreman
The Carpenter Foreman shall be responsible for the direct execution of works
involved in the formwork according to the plan, and shall directly control and give
instruction to the working personnel. He is responsible to oversee the proper use of
tools and personal protection equipment, and housekeeping in the carpentry
fabrication shop and at site.
Safety Manager / Supervisor
He shall review prior to the start of the work Safety Action Plan that must be
implemented and observed in relation with formwork during the construction. He
shall review the applications for work permits if required from safety point of view
before submitting to AMCI.

8.0 WORK PROCEDURE
1. Secure reference IFC drawing regarding the shape, size and dimension
of the forms required.
2. Fabricate the forms as per the requirement specified in the project
specification ,Concrete Construction and in accordance with ACI 347 and
tolerance limit listed in ACI 301, using either 12mm or 15mm thick marine
plywood and ribbed wit 2”x4” timber. Form frame shall have longitudinal
ribs, And ribs shall be free from deformities and defects to resist buckling
effect when subjected to enormous dead load of concrete.
3. Apply form-releasing agent approved prior to fixing of any formworks in
use.
4. Fix 25mm plastic chamfer strips to exposed exterior corners of the
concrete structure or as specified on the construction drawing.
5. Assemble and fix the forms to the correct position as marked by the
Surveyor. Check the dimension to conform to the required size and
shape of the concrete member, subject to the tolerances prescribed in
the project specification.
6. Secure the forms to its position properly by using tie rods, clamps, or
wood cleats or combination of which that is suitable depending on site
condition. Forms must be watertight to prevent run-off of cement paste
from the fresh concrete and to avoid honeycombs.
7. Use approved waterproof through-tie system for underground structures,
especially for pits.

8. Wall forms shall be in layer of sectional units. The joints formed by the
assembly of these sectional units must be fitted well, aligned and
secured properly.
9. In the case of overhead formworks, make sure that the scaffolding and
other part of the staging are properly fixed and have passed the safety
requirements.
10. Provide bracings on sides of forms to prevent any movement or
displacement of any part during the pouring of concrete using 4”x4”
timber or mechanical form jacks with ends anchored on solid and stable
base. For overhead formwork, bracings can be anchored on part of the
scaffold found suitable.
11. Check and correct any misalignment or displacement resulting during the
fixing of braces and supports. Check verticality by attaching plumbs on
the sides of the forms or by using aluminum spirit level.
12. Forms for rectangular and square shaped structures shall be maintained
at right angle.
13. Mark the top of concrete elevation by fixing nail on two opposite sides
inside the form and on starter bars for wider areas of pouring.
14. Clean the inside forms. For deep forms, provide an opening at the
bottom of the formworks big enough for dirt, small wood pieces and other
debris to pass through when air blown.
15. Deep formwork shall have a cut out / window opening at every 1.5 meter
and side chutes or provide extension to boom of concrete pump like
flexible hose or funnel to eliminate higher drop of concrete and concrete
segregation.
16. Forms shall be free of noticeable wrinkles, warps, big patches, and other
such defects. Contact surface of forms shall be clean, and free of ruts
and other roughness.

17. Determine the wind velocity during installation of wide and big pre-
fabricated formworks and stop when the wind speed dictates that its
unsafe to do lifting works.
8.1 Removal of Forms
1. Forms for columns, walls, sides of beams and other vertical forms
not supporting concrete weight maybe removed 48 hours after
placing, if the concrete has hardened sufficiently.
2. Forms for beam soffits, slabs, and other parts requiring support
shall remain in place until the concrete has attained 60% of its
specified 28-day strength. This strength is to be verified by the
crush test results of 7day break and the test cylinder is to be the
FIELD CURE SPECIMEN not the tank-tested cylinder.
3. Remove the bracings and supports carefully to avoid inflicting or
causing damage to the concrete.
4. Remove first all the side support and bracings followed by the
clamps and tie rods.
5. Using a crowbar, strip the forms carefully in a manner that will not
cause damage to concrete surface.
6. Clean the removed forms of any hardened concrete. Remove any
protruding nails, wood and plywood spurs upon removing the
formwork.
8.2 Coating Preparation:
1. Where coatings are to be applied all NAILS and other protrusions
are to be totally removed.
2. Concrete is to be prepared as required by the coating
Manufacturer in all cases.

9.0 Inspection
The QC Inspector of KCC and AMC Inspector shall carry out the necessary
inspections in various stages of the formworks. Inspections shall be based
on prepared ITP and Project Specifications listed in References of this
Method Statement.
10.0 Safety Plan
1. KCC Safety Supervisor shall carry out the safety site inspections to ensure that
safety requirements laid out in the Safety Action Plan are followed during the
execution of works.
2. KCC Safety Manager shall review the prepared Safe Action Plan and secure
Work Permit if required prior to start of any activities.
3. Personal Protection Equipments shall be provided to all personnel involved in
the fabrication and installation of formworks.
4. All personnel involve in the work shall have to go undergo Safety Orientation
being conducted by AMC.
5. Barricades shall be set up around the work sites. Warning signs shall be
posted to caution other personnel on site of the on going formworks.

10.2 Equipment, tools and materials inspection.
1. Power hand tools shall be checked of any loose and exposed wiring. It shall
be marked with color- coding tape signifying compliance to safety inspection
requirement. All power tools shall be rated at 110 V only.
2. Hand tools shall be inspected frequently for any damages. Defective tools
shall be removed from service and repaired or disposed. Hand tools also
shall be color-coded.
10.3 Material Safety Data Sheet
Material Safety Data Sheet shall be provided on site, where material
classified as hazardous, is being used. It shall be submitted to MHI along
with the Material Approval Request for review and approval to use in the
project.
