1.0 Purpose and Scope

1.1 This document describes the principals of the construction method, measures inspection,
quality control and testing scheme for (Form work) works for the contract construction of
the project as well as the main equipment which is anticipated for the execution of the
work.
1.2 Applicable to all SUB ELEMENT and MAIN ELEMENT constructed in the project.

2.0 REFERENCE

3.0 PERSONAL RESPONSIBILITIES

A full and detailed description of the roles and responsibilities of all (Ecologi) staff is included
within the Project Quality Plan. For the works as covered by this method statement, the following
personnel have specific duties.

3.1 Technical office Engineer specific responsibility include :


 Responsible for ensuring that the various submittals are timely undertaken
(materials, method statements, workshop drawings, subcontractors issues, technical
issues)
 Responsible for quotations as required (materials, subcontractors).
 Responsible for ensuring RFI’s are timely undertaken on behalf of (Ecologi)
Responsible for the distribution of documents within (Ecologi) site personnel.

3.2 HSE Site reprehensive specific responsibility include:


 Assisting in the production of any health, safety and environmental documents.
 Monitoring the implementation of health, safety and environmental requirements.
 Undertaking audits and ensuring that corrective and preventive actions are taken
where required.
 Liaison with the senior site staff on health, safety and environmental related matters.

3.3 Site Engineer specific responsibility include:


 Ensuring that the project is managed in a safe manner and meets the requirements of
the project documentation.
 Ensuring that subcontractors and suppliers achieve the on-site required standards.
 Shall refer to the QA/QC Department and the HSE Department for advice and
guidance when required.
 Shall ensure that operatives comply with the relevant method statements, Toolbox
Talks and general site briefings.

3.4 QA/QC Site representative(s) specific responsibility include:


 Carrying out phased inspections as required.
 Preparation of the ITP.

 Ensuring that all required tests are performed and are in compliance with appropriate
specifications.
 Maintaining a record of all QA/QC activities on site.
 Controlling and ensuring all measuring and test equipment on site is properly
calibrated.
 Controlling all non-conforming products or workmanship, and implementing the
required preventive actions.
 Ensuring that corrective and preventive actions are taken when required.
 Ensuring that the required testing is undertaken by approved independent
laboratories, and that all test results are reviewed.

3.5 Supervisor (s) specified responsibilities include


 To ensure that all operatives and appropriate subcontractors have been briefed as to
the content of the method statement and the works.
 To adequately control the works as required.
 To ensure the works are undertaken in a safe manner.
 To ensure all operatives are adequately training in their respective works.
 Reporting any un-safe practices on site to the (Ecologi) site team.

4.0 EXECUTION PROCEDURE


4.1 All formwork activities will be carried out in accordance with the specification.
4.2 After studying the structural drawings, the Project Engineer will plan for sizes, numbers and type
of formwork to be used in consultation with the Structural Engineer, if required. Depending upon the
programme, the number of repetitions will be decided.
External designers may be used for specific systems.
4.3 Availability of formwork will be checked with the Procurement Manager and decision will be
taken to buy, hire or reuse formwork material.
4.4 During fabrication of formwork shutters, all dimensions shall be cross-checked with a view to
avoiding corrective action during erection.
4.5 Paint reference numbers on all panels to ensure their use in correct positions.
4.6 Ensure that the props, shores, waling, bearers, clamps and tie rods are the right size and at the
correct spacings. The Formwork designer shall check the falsework system with respect to load
imposed on it and design parameters as laid down in the specification.
4.7 Check that the falsework is securely braced and is on a firm foundation.
4.8 Forms fastened to previously cast concrete must be tightly fixed to prevent grout loss. Cellular
foam plastic strips can be used to make a seal.
4.9 Check quality of shutter lining, tightness of bolts and wedges, built-in items, inserts and other
embedment’s.
4.10Tie – rod holes to be made in the formwork shall be neat so that they can be patched or plugged
later on.
Particular attention will be paid to the rigidity and line of stop ends and joint formers.
4.11 Remove all tie-wire clippings and nails, which may stain both the formwork and the concrete.
4.12 Ensure that adequate access and working platforms are in place for the concreting gang and that
toe boards and guardrails are provided.
4.13 A spreader or lifting beam shall be used to prevent distortion when placing formwork.

4.14 Adjustable steel props shall be erected in plumb and verticality of props checked using a spirit
level.
4.15 A prop should not be used if it is bent, creased or, rusted or has a bent head or base plate,
damaged pin etc.

4.16 Proper release of agent shall be used so that it can be removed without damaging the surface
finish of the concrete.
4.17 New timber and plywood forms shall be given first coat of the appropriate release agent 36 hours
in advance, then a second coat just before they are used. Avoid excessive use of release agent as this
can cause staining or retardation of the concrete.
4.18 A close watch should be kept on all wedges and fastenings. Make sure that they do not work
loose. Grout loss is an indication that some movement has occurred.
4.19 When unsleeved tie bars are being used, ease them slightly before the concrete sets, and remove
them as soon as it is safe.

Striking the Formwork:

i) Always obtain approval before striking the formwork in the form of Appendix CONC/04/A.

ii) Ensure adequate safety measures while striking formwork.

iii) Cleaning the face of forms shall start as soon as they have been struck.

iv) Good storage is essential to ensure that formwork can be used again.

v) Paint code numbers on them so that they can be readily identified when next needed.

4.20 Safety During Formwork Operations:

i) The weights of large prefabricated sections of formwork should be marked on them so that the
lifting capacity of the crane at the working radius is not exceeded. Lifting points are usually provided.

ii) If necessary, a spreader or lifting beam shall be used to prevent distortion.


iii) Where lifting is done by the vertical soldiers, check that they are adequately connected to the
walings.

iv) Fire extinguishers in working order shall be in close proximity to the stored formwork.

6.0 QUALITY CONTROL


Quality control staff will ensure performing all work in accordance with Project technical
Specifications and General Quality Control Plan as follow:
● Each activity should be performed by the assigned employee within the organization chart.
● All materials and equipment used in the activities should be approved, inspected and tested
prior to usage.
● Work procedure should follow this method statement and preparatory technical meeting.
● All activities steps should follow ITP1.
7.0 SAFETY
9.1 Safety requirements will be addressed in accordance with the Project General Safety Plan.
9.2 Ensure that sides of excavation will be protected and soil stability of the surrounding areas
will be secured.
9.3 Barricade the excavation area with warning tapes and install Safety signs and notices to
create Safe Working area.
9.4 Concrete pump shall extend their out riggers fully during pouring of concrete, Pumps
discharge hose shall be properly connected & bearing pad to be placed below the wheels of
pumps at the time of pouring.
9.5 Tools and equipment will be inspected periodically by the Contractor to ensure that remain
in proper working order and calibration certifications is current.
9.6 During night works, a suitably trained supervisor will be on site for the full night duration
and adequate lighting shall be provided.
