1. PURPOSE & SCOPE

The objective of this method statement is to provide general guidelines for “Erection and Inspection of Formwork at site. This method statement covers the
activities of formwork at all project sites.

Formwork

Project :

Project/Construction Manager
Overall implementation and monitoring of all the activities in the method of statement of the formwork.

Site Manager

Site Manager will be responsible:-

Over-all direction

Monitoring

Coordination

Control of all the constructions activities related to the formwork works

Site Engineer

The Site engineer will be responsible:-

Directly involved in the planning and execution of the activities involved in the formwork/shuttering works

Preparation of the shop drawing

Safety action plan and safe work plan,

Application for work permits, if required prior to commence formwork/shuttering works

QA/QC Engineer

QA/QC Engineer will be responsible:-

Monitoring tests to be carried out as per the approved ITP.

Coordinating with the independent testing laboratory for the testing.

Check formwork/shuttering as per approved ITP & IFC drawing.

Foremen

Foremen will be responsible:-

Direct execution of works involve in the formwork/shuttering works according to the plan.

Directly control and give instruction to the working personnel.

Ensure that works are performed with proper tools/equipment, PPE’s and good housekeeping around the working area.

HSE engineer/Supervisor

Safety engineer / Supervisor will be responsible:-

Review prior to the start of the work Safe Work Plan and work permit,

If required a submittal to client/customer that must be implemented and observed during the steel rebar/reinforcement works.

5. TOOLS & EQUIPMENT

Cutting disc, jacksaw, Hammer/stiker, Pin/nail puller

Project :

Formwork material shall confirm to ACI 347R and shall be of wood, steel, driven sheet piling or other approved material from client/consultant. The type, size,
shape, quality and strength of the material of which the forms are to be made shall be consistent with the specified finishes and safety.

Inspection test plan (ITP) specific to project requirements shall be made and relevant inspection activities shall be recorded either on the company formats
or those supplied by Customer.

IFC drawings shall be consulted for concrete work carefully and shop drawing shall be prepared for formwork/shuttering which include shape, size,
dimension and support assembly like frame for the formwork, scaffolding pipe, joints, shuttering plates etc.

Formwork shall be strong, stable and rigid to withstand the load of the fresh concrete during the pouring and to keep or maintain its position and shape.

Formwork/Shuttering shall be positioned to correct lines and must be in plumb & level conforming to shape, lines, elevations and dimensions of concrete
shown on the IFC drawings.

The faces of the forms shall be true flat and free from any undulation/corrugation that may affect the finished concrete.

Chamfer strips of 20 mm shall be used or approved on all exposed edges as shown on IFC drawings.

Support and brace all forms adequately so as to maintain the desired position during and after the placing of concrete and build sufficiently tight to prevent
mortar leakage.

Before placing any reinforcement the forms surfaces must be cleaned and coated with approved form of release agent.

Fix foam tape to all formwork joints and edges to prevent the loss of cement slurry.

Position forms accurately to tolerance and firmly secures anchors, bolts, inserts, supports, hangers, pipe sleeves, drains, water stoppers etc. before concrete
is poured.

The forms shall be removed as per project specification/requirements. Form sheets shall not be thrown down to the ground. Extreme care shall be taken so
as not to damage the formwork.

After the careful removal of the form sheets the same shall be taken to the site workshop for minor repairs and oiling.

Form sheets shall be properly segregated according to the size and stored under the shelter for onward usage.

Submit request for final checking/approval by Customer/Consultant as required.

In case of non-conformity at any stage of inspection the segregation shall be covered by method of statement and Corrective Action Request (CAR) shall
be raised as per client/customer laid down procedure
