AFFIDAVIT OF LOSS

I, ALEXANDER ESPINO BLANCO, Filipino, of legal age and a resident of 2352


SAMALA-MARQUEZ, BINAKAYAN, KAWIT, CAVITE Philippines, after being duly
sworn in accordance with law, depose and state:

That I am a regular subscriber / user of GLOBE PREPAID Mobile SIM Card with
No. 09262123290;

That in the afternoon of DECEMBER 24, 2017, I forgot my Iphone 6 cellular


phone with the said SIM Card in the pocket side of my motorcycle which I have
parked along Cavite City;

That when I immediately returned, I discovered that my cellular phone was


missing together with my said Mobile SIM Card which was inside the said cellular
phone;

I failed to recover my cellular phone and the said SIM Card despite diligent search
and earnest efforts to contact and locate said phone, and I consider the same as lost and
can no longer be found;

I am executing this Affidavit to attest to the truth of the foregoing facts and in
support of my application for the replacement of the said SIM Card and for whatever legal
intents and purposes it may serve.

Affiant states nothing further.
