AFFIDAVIT OF LOSS

I, LUZ T. BILAG, of legal age, married, presently residing at Villa, Sta. Teresita, Cagayan,
after being duly sworn, hereby depose and state:

1. That I am the owner and pawner of an item described in the pawnticket as 18k, 1.80gms
Necklace with Pendant Gold, which I pawned at CEBUANA LHUILLIER
PHILIPPINES, Cebuana Lhuillier Sta. Teresita Branch, and for which I was issued
Pawnshop Ticket No. 60275sg012960 with Maturity Date on November 21, 2019 and
Expiry Date of Redemption Period on February 19, 2020;

2. That the pawn receipt issued to me by CEBUANA LHUILLIER PHILIPPINES, was


unfortunately lost or misplaced by me;

3. That despite all efforts to find the said pawn receipt, I still cannot locate it and consider
it lost forever;

4. That I am executing this affidavit to attest to the truth of the foregoing facts and to use
the same for whatever legal purpose it may serve.
