AFFIDAVIT OF UNDERTAKING

I, RICHELL A. TABULA, of legal age, Filipino citizen, married, and a


resident of Villa, Sta. Teresita, Cagayan, after being sworn to in accordance with
law, depose and state that:

1. That I am the mortgagor/authorized driver of a motor vehicle which is more


particularly described as follows:

2. That the said vehicle was involved in an accident sometime on 2016 at


Mission, Sta. Teresita, Cagayan;

3. That the above-mentioned motorcycle collided with another motorcycle;

4. That as result of the collision both vehicles were damaged;

5. That the aforementioned vehicle was impounded by the authorities at PNP


Police, Station, Sta. Teresita, Cagayan;

6. That I hereby undertake to produce the said vehicle upon request of the
authorities if such will be needed for further proceedings or investigation;

7. That this affidavit is being executed to support my request for the release of
the impounded vehicle;

8. That I am executing this affidavit to attest to the truth of the foregoing facts.
