AFFIDAVIT OF BODY COLOR

I, TEODORICO T. PICARDAL, of legal age, married and a resident of Sibulan,


Countryhomes, Maslog, Sibulan, Negros Oriental, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That I am the owner of a motor vehicle more particularly described below;

MAKE : TOYOTA TAMARAW WAGON


ENGINE NO : 7K-0231783
CHASSIS NO : KF80-8001568
PLATE NO : GML800

2. That the above-described VEHICLE’s Body paint when I purchased it originally


SILVER GRAY, unfortunately it was not indicated in its registration.

3. That I have not change its color and it still was painted with SILVER GRAY
color;

4. That I am executing this affidavit to attest to the truth and veracity of the foregoing
declarations and to indicate the color Silver Gray in certificate of registration from none and
to change the color RED in the Official Receipt to SILVER GRAY.

IN WITNESS WHEREOF, I have hereunto set my hand this 11th day of December
2020, at Dumaguete City, Negros Oriental, Philippines.
