AFFIDAVIT OF LOSS

I, JUNREY C. GEMARINO, of legal age, Filipino citizen and a resident of Sta.


Maria, Zamboanga City, Philippines, after having been duly sworn to in accordance with
law, hereby depose and say, that:

1. I am a holder of a Certified Public Accountant Identification Card issued by


the Professional Regulation Commission (PRC for brevity) in Zamboanga
City;

2. However, on March 2020, I have discovered that said PRC ID got lost and
cannot be located;

3. I exerted diligent efforts to look and locate for the above-mentioned PRC ID in
other places, but all such efforts proved futile and to no avail;

4. For all intents and purposes, I am now declaring the above-mentioned PRC
ID lost and beyond recovery;

5. I execute this Affidavit to attest to the truth of the foregoing facts based on my
personal knowledge and authentic records in compliance with the
requirement of the PRC, Zamboanga City in lieu of the lost one and for
whatever legal purposes it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand on this 28 th day of


December 2020 at Zamboanga City, Philippines.
