AFFIDAVIT OF LOSS

KNOW ALL MEN BY THESE PRESENTS:

I, FATIMA MENDING - ABAS, of legal age, Filipino, married, and a


resident of #003 Albert Einstein Road, Rosary Heights 7, Cotabato City, after
having been sworn in accordance with law, hereby depose and state the
following:

That I am the same person executing this Affidavit;

That I am the holder of Tax Identification Card with ID No. 194-563-


247 issued by Bureau of Internal Revenue;

That on November 22, 2019 during the PICPA National Convention at


Laoag City, Ilocos Norte, I lost the said TIN Card;

That despite diligent search and efforts to locate the said card, the
same could not be found such that I now consider it lost beyond recovery;

As such, I am executing this Affidavit of Loss to attest to the truth of


the foregoing and for any legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 30th day of


December, 2019 at Cotabato City.
