AFFIDAVIT OF LOSS

I, EMMANUEL TAN, of legal age, Filipino citizen with office address at


Bangayan Compound, Veterans Avenue, Zamboanga City, Philippines, a duly
authorized representative of POWER EXPRESS FORWARDER, INC., with business
address at Door No. 5 Bangayan Compound, Veterans Avenue, Zamboanga City,
Philippines, a domestic corporation duly existing in the laws of the Republic of the
Philippines, after having been duly sworn to in accordance with law, depose and say,
that:

1. I am the Operation Manager of Power Express Forwarder Inc., a domestic


corporation duly existing in the laws of the Republic of the Philippines, engaged
in the business of freight forwarder;

2. Relative to my work, I issued receipts after payment made by PRICEWISE


MARKETING CORPORATION. Said receipts were under my custody and usually
kept together with other office files;

3. However, due to inadvertence and oversight, I recently discovered that some of


the receipts were no longer where I kept it. Particularly the receipt series with
reference numbers: CR98329ZAM, CR98330ZAM, CR98336ZAM,
CR98338ZAM, CR98327ZAM, CR98323ZAM, CR98322ZAM;

4. I exerted diligent efforts to locate for the same in other places but all such
endeavors proved futile and to no avail;

5. For all intents and purposes, I am now declaring that the above-enumerated
receipts were lost and beyond recovery;

6. I execute this Affidavit to attest to the truth of the foregoing facts based on my
personal knowledge and authentic records for whatever legal purpose it may
serve.
